Overview
Population Health Essentials outlines the fundamentals necessary to improve outcomes and reduce costs. Through case studies and real-life workforce applications, participants will review and analyze key areas of population health. Participants will learn how a population health perspective affects healthcare priorities in the areas of transitions of care/care coordination, prevention, public health and health policy and how implementing a population health strategy can improve health outcomes, reduce costs, improves provider and patient engagement and satisfaction.


Objectives
At the conclusion of this activity, learners will be able to:

  1. Examine the role of key stakeholders and their complex interrelationships on population health outcomes in the US healthcare system.
  2. Describe the population health paradigm
  3. Identify models and strategies for population health management and creating a culture of wellness
  4. Examine the economic concept of risk as it applies to healthcare
  5. Discuss the role of quality improvement in the context of population health management
  6. Apply risk management theory to population health management
  7. Discuss the impact that social determinants of health have on health outcomes
  8. Explain the necessity for data management and analytics for population health, in care delivery, care coordination, patient engagement and organizational decision making
  9. Define models of care coordination and care management that proactively manage patient care across the continuum.
  10. Describe how diverse healthcare professionals can collaborate and use shared resources to develop strategies that meet the specific health and healthcare needs of their patients and populations.
  11. Discuss interventions that reduce the impacts of SDOH, improve health outcomes and decrease health care costs.

Successful Completion
To successfully complete this activity, the required evaluation must be completed no later than Saturday, April 15, 2023. Thereafter, the evaluation will no longer be available and credits will not be issued. No refunds will be offered for failure to complete the required evaluation by the stated deadline.


Accreditation

 

In support of improving patient care, Thomas Jefferson University is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.

Physicians: This educational activity is designated for a maximum of 17.00 AMA PRA Category 1 Credits TM . Physicians should only claim credit commensurate with the extent of their participation in the activity.

Cancellation Policy

Requests for refunds must be submitted in writing and received in the Office of CPD two weeks prior to the start date of the course. There will be no refunds after this period.*  Cancellations notified before the two week time frame will incur a 25% administration fee (with a minimum of $20).  Registrants who fail to attend the conference are responsible for the entire fee.  All refunds will be processed after the conference.  The University reserves the right to cancel or postpone this course due to unforeseen circumstances.  In the event of cancellation or postponement, the University will refund registration fees, but is not responsible for related costs or expenses to participants, including cancellation fees assessed by hotels, airlines or travel agencies.
