This unique training conference will provide an intensive, collaborative, and engaging experience to providers, educators, and leaders across health, education, and social disciplines, as well as to community members invested in promoting health of their families, neighborhoods, and cities. This conference is centered on the combatting social determinants of health.

For Act 48- Act 48/PERMS credit  is provided by The University of Pennsylvania Graduate School of Education.  

Successful Completion
To successfully complete this activity, the required evaluation must be completed no later than Wednesday, August 7, 2024. Thereafter, the evaluation will no longer be available and credits will not be issued. No refunds will be offered for failure to complete the required evaluation by the stated deadline.


Objectives
At the conclusion of this activity, healthcare team members will be able to:

  1. Identify the unique mental health and physical health needs of patients who have experienced potentially traumatic events.
  2. Analyze how trauma-informed care principles are embedded into their current policies and practices within their current professional role.
  3. Develop a plan to infuse trauma-informed care principles into their current workplace policies and practices.
  4. Identify three new patient engagement practice that you will infuse into your professional practice.

In support of improving patient care, Thomas Jefferson University is jointly accredited by the Accreditation Council for Continuing Medical Education (ACCME), the Accreditation Council for Pharmacy Education (ACPE), and the American Nurses Credentialing Center (ANCC), to provide continuing education for the healthcare team.

 

Physicians: This educational activity is designated for a maximum of 11.00 AMA PRA Category 1 Credits TM . Physicians should only claim credit commensurate with the extent of their participation in the activity.
